What Is Wisdom Tooth Extraction and When Is It Needed?
June 17, 2026Wisdom tooth extraction is the removal, under local anesthesia through surgical or standard methods, of third molars that remain fully or partially impacted in the jawbone or that erupt at the wrong angle and fail to integrate properly into the mouth.
It's a misconception that every wisdom tooth must be removed. If a tooth has erupted at the correct angle, contributes to chewing and can be cleaned properly, it can stay in the mouth. However, extraction becomes unavoidable in the following situations:
- Pericoronitis (Gum Inflammation): Food debris gets trapped in the gum pockets around a partially erupted tooth. Since this area can't be fully cleaned, it leads to foul-smelling inflammation, difficulty opening the mouth (trismus) and trouble swallowing.
- Decay in Neighboring Teeth: A wisdom tooth erupting at an angle presses against the second molar in front of it. Since the area between the two teeth can't be brushed, it silently causes decay in the otherwise healthy neighboring molar.
- Orthodontic Reasons: The pressure these teeth exert on the jawbone can cause crowding in the front teeth, or extraction may be planned to protect the results for patients undergoing braces or clear aligner treatment.
- Cyst or Tumor Formation: The protective follicle around some fully impacted wisdom teeth can develop into a cyst over time, causing bone resorption and structural weakness in the jaw.
Surgical Comfort at Dent Ümitköy: How Is a Wisdom Tooth Extracted?
Fear of surgery is one of our patients' biggest concerns. At Dent Ümitköy, we complete the entire surgical process with maximum comfort for our patients through advanced diagnostic methods and pain-free anesthesia techniques:
- 3D Dental Tomography and Millimeter Planning: Wisdom tooth roots can sit very close to the main sensory nerve running through the lower jaw (inferior alveolar nerve) or to the sinus cavities in the upper jaw. With our 3D dental volumetric tomography (CT), we view the tooth's position and its proximity to the nerve to the millimeter, reducing surgical risk to zero.
- Local Anesthesia and Isolation: The surgical area is fully numbed with modern local anesthesia techniques. Patients feel no pain or aching throughout the procedure — only a sensation of pressure and touch.
- Removing the Tooth and Bone Relief: If the tooth is impacted, the gum is opened with a small incision. The thin layer of bone covering the tooth is carefully relieved with special instruments. If the tooth cannot come out whole, it is sectioned into pieces (the separation method) to safely remove it without damaging the jawbone.
- Wound Cleaning and Suturing: After extraction, the socket is thoroughly cleaned and rinsed to prevent infection risk. The gum tissue is closed with cosmetic sutures, which are typically checked and removed by our dentist after 7 to 10 days.
An Alternative for Anxious Patients: Extraction Under Sedation
For patients with an intense fear of needles or surgery, a strong gag reflex that makes it hard to tolerate instruments, or who want all four wisdom teeth removed in a single session, Dent Ümitköy offers sedation (deep sleep). Administered under the supervision of our anesthesiology specialist, this method completes your entire procedure while you're in a light sleep, and you won't remember any of the unpleasant details once it's over.
Golden Rules to Follow After Wisdom Tooth Extraction
For a successful outcome and quick recovery, there are critical rules to follow in the first 48 hours after extraction:
- Biting Down on the Gauze: The sterile gauze placed to stop bleeding should be bitten down on firmly for at least 30–40 minutes, then discarded.
- Don't Spit: Saliva and any light blood should be swallowed, never spat out. Spitting creates a vacuum effect in the mouth that can dislodge the clot and restart bleeding.
- Diet: Avoid very hot, very cold, acidic and grainy foods (rice, sesame, etc.) on the first day. Choose lukewarm, soft foods like soup, yoghurt and purées. Never use a straw.
- Ice Packs: Applying ice intermittently to the outside of the cheek during the first 24 hours (5 minutes on, 5 minutes off) significantly reduces post-operative swelling and bruising.
- No Smoking: Cigarette smoke disrupts clotting and can cause a severe, painful inflammation at the wound site known as dry socket (alveolitis). Avoid smoking for at least 72 hours after surgery.
