What Causes Gum Disease? The Hidden Triggers

Gum disease, also known as periodontal disease, refers to infections affecting the tissues surrounding the tooth and the jawbone that holds the root. This process doesn't develop suddenly — it stems from bacteria accumulating over time and the body's immune response to them.
Poor oral hygiene is responsible for the vast majority of cases. Food debris left in the mouth combines with bacteria to form a soft plaque on tooth surfaces; if not cleaned regularly, this plaque absorbs minerals and hardens into tartar. Bacteria in tartar that can't be removed by brushing irritate the gums, causing redness, swelling, and bleeding.
Smoking is one of the biggest accelerators of gum disease; it narrows capillaries, impairing gum nourishment and reducing resistance to infection. Smokers may not experience bleeding despite inflammation, allowing the disease to progress unnoticed. In diabetic patients, uncontrolled blood sugar increases susceptibility to infection and accelerates bone loss; the relationship is bidirectional — treating gum inflammation also helps regulate blood sugar.
Hormonal fluctuations during pregnancy, puberty, and menopause increase gum sensitivity to bacteria. People with a family history of early tooth loss may have a genetic predisposition. Chronic stress and poor nutrition also weaken the immune system, causing gum tissue to break down more quickly.
As the disease progresses, it passes through three stages: reversible gingivitis with no bone loss, periodontitis where inflammation reaches the bone, and advanced periodontitis where teeth loosen and are lost. A problem caught in its early stage can be fully resolved with regular tartar removal.
